Heart attack versus cardiac arrest: plumbing issues vs. electrical failures.

Gold Coast, Sep 9 (The Conversation) – In July 2023, Bronny James, an emerging figure in US basketball and son of renowned LA Lakers veteran LeBron James, experienced a life-threatening incident during practice when he collapsed due to a cardiac arrest. The 18-year-old was rushed to the hospital following this critical incident.

Miscommunication surrounding this event arose as various media outlets inaccurately labeled the situation as a “heart attack,” often using the terms interchangeably. Understanding the distinction between a cardiac arrest and a heart attack is crucial, as both pertain to the heart but represent different medical conditions.

### Understanding the Heart’s Function

The heart is a vital muscular organ responsible for pumping blood throughout the body. As it contracts, it distributes oxygen and nutrients to various tissues, relying on a network of coronary arteries that supply it with blood. When these arteries become obstructed, the heart itself may not receive the necessary blood supply, resulting in potential damage or even death of the heart muscle, subsequently impairing its ability to pump effectively.

### Differentiating Between Heart Attack and Cardiac Arrest

A heart attack, clinically known as a myocardial infarction, refers specifically to the damage or death of heart muscle tissue. Conversely, cardiac arrest, often termed sudden cardiac arrest, occurs when the heart abruptly stops beating effectively. While both conditions represent a failure of the heart to function correctly, they stem from different causes and disease processes.

### Causes and Risk Factors

Heart attacks typically arise from blockages in the coronary arteries, a condition commonly referred to in Australia as ischemic heart disease. About 75% of heart attack cases are associated with atherosclerosis, a gradual buildup of fatty and fibrous deposits within the artery walls, leading to plaque formation. This plaque buildup can limit blood flow or foster the generation of blood clots.

Numerous risk factors contribute to the development of atherosclerosis, including high blood pressure, elevated cholesterol levels, obesity, diabetes, stress, and genetic predisposition. Additionally, heart attacks can be triggered by artery spasms, direct trauma to the chest, or other factors that impede blood flow.

On the other hand, cardiac arrest results from irregular heartbeat patterns caused by electrical dysfunctions within the heart. Common arrhythmias linked to cardiac arrest include:

1. **Ventricular tachycardia:** An excessively rapid heart rhythm over 100 beats per minute, preventing the heart from filling adequately with blood.
2. **Ventricular fibrillation:** A chaotic quivering of the heart, often exceeding 300 beats per minute, impeding its pumping action.
3. **Pulseless electrical activity:** Occurs when the heart cannot generate enough force to maintain a pulse, despite having electrical activity.
4. **Asystole:** A collapse of electrical activity in the heart, commonly depicted as a flatline on monitors.

Cardiac arrests can be triggered by various factors, including drowning, trauma, asphyxia, electrical shocks, and drug overdoses. In the case of Bronny James, his cardiac arrest was linked to a congenital heart defect, a condition he was born with. Notably, ischemic heart disease, such as from a heart attack, is responsible for around 70% of all cardiac arrest cases.

### The Link Between Heart Attacks and Cardiac Arrest

The connection between heart attacks and cardiac arrest is significant. Damage to heart tissue during a heart attack can disrupt its ability to conduct electrical signals, increasing the likelihood of arrhythmias, which may eventually lead to a cardiac arrest. Nevertheless, while heart attacks are a frequent cause of cardiac arrest, the reverse is not true; a cardiac arrest typically does not induce a heart attack.

### Recognizing Symptoms

Symptoms of cardiac arrest manifest suddenly, often leading to a loss of consciousness, absence of a pulse, cessation of breathing, and a pale or bluish skin appearance. In contrast, the symptoms of a heart attack can vary; common indicators include chest pain or discomfort that may radiate to the arms, back, neck, jaw, or stomach. Additional symptoms may involve shortness of breath, nausea, light-headedness, paleness, and sweating.

### Conclusion

While both cardiac arrest and heart attacks relate to heart health, they differ fundamentally in their causes and impacts. A heart attack could be likened to a blockage in plumbing, whereas cardiac arrest parallels an electrical malfunction in a building. Regardless of their differences, both conditions necessitate swift medical intervention to prevent severe outcomes.
